Amphotericin B
Indications/Uses
Potentially life-threatening fungal infections.
Dosage/Direction for Use
0.1 mg/mL conc administered as slow IV infusion over 2-6 hr. Initially 0.25 mg/kg daily over 2-6 hr. Patient w/ good cardiopulmonary function & tolerance w/o severe reaction Initially 0.3 mg/kg daily. Impaired cardiopulmonary function or severe reaction Smaller initial dose of 5-10 mg daily. May be increased gradually by 5-10 mg daily to 0.5-0.7 mg/kg daily. Max total daily dose: 1.5 mg/kg. Treatment duration: 6-12 wk or longer.
Contraindications
Hypersensitivity. Hepatic disease.
Special Precautions
Fever, chills, nausea, vomiting, anorexia, headache, arthralgia & hypotension. Frequently monitor liver & renal function, serum electrolytes (particularly Mg & K) & blood counts.
Adverse Reactions
Fever, malaise, wt loss; anorexia, nausea, vomiting, diarrhea, epigastric pain; normocytic anemia, inj site phlebitis; muscle & joint pains; headache; decreased & abnormal renal function including decreased CrCl, hypokalemia.
Drug Interactions
Enhanced potential for renal toxicity w/ other nephrotoxic drugs eg, cisplatin, pentamidine, aminoglycosides & cyclosporine. Potentiated induced hypokalemia w/ corticosteroids & corticotropin. Increased toxicity of digitalis glycoside, muscle relaxants & antiarrhythmics.
